Terms and Conditions of Use - Soccer Class Enrollment Request

1. Acceptance of Terms
By completing and submitting the enrollment request form for soccer classes, the user (hereinafter referred to as "Student" or "Guardian") acknowledges that they have read, understood, and agree to all the terms and conditions outlined in this document. If the Student or Guardian does not agree with any of the terms, they should not complete the request process.

 

2. Nature of the Request
This form serves as a request for enrollment and does not guarantee confirmation of enrollment in the soccer classes. The final enrollment will depend on three conditions:

  • Availability of spots in the requested classes.

  • Profile review by the organization, assessing the suitability for participation.

  • Signing of a contract for services, and

  • Payment confirmation for the registration or tuition fee.

Enrollment will be confirmed only after the profile review, availability of spots, contract signing, and payment are completed.

 

3. Personal Data and Accuracy of Information
The Student or Guardian agrees to provide truthful, complete, and up-to-date information in the enrollment request form. If there are any changes to the provided information, the Student or Guardian must update it immediately to ensure accuracy during the request and review process.

 

4. Review Process and Enrollment Confirmation
After submitting the request form, the organization will review the Student's profile, considering spot availability and suitability for the classes. The Student or Guardian will be notified by email or message regarding the status of the request.

  • If the request is approved, the Student or Guardian will receive an email with instructions for contract signing and payment.

  • Enrollment will be confirmed only after the positive review of the profile, contract signing, and payment confirmation, subject to availability of spots.

 

5. Contract Signing
Signing a contract is mandatory to formalize enrollment and ensure participation in the soccer classes. The service contract will be sent to the Student or Guardian after the enrollment request is approved. Enrollment will be considered finalized only after the contract is signed within the deadline specified by the organization.

 

6. Health Responsibility
The Student or Guardian affirms that the participant is in good physical condition to participate in the soccer classes and has no medical contraindications for the sport. The organization is not responsible for any injuries or health issues that may occur during the classes, once enrollment is confirmed.

 

7. Payment and Fees
After the contract is signed, the Student or Guardian must make the payment for registration or tuition fees as informed by the organization. Payment must be made within the specified deadline to ensure enrollment and participation in the classes. Enrollment will be confirmed only after the contract is signed and payment is confirmed, subject to spot availability.

 

8. Cancellation and Refund Policy
In case of withdrawal after enrollment is confirmed, the Student or Guardian must follow the organization’s cancellation and refund policy. Refunds will be processed according to the established terms, which may be either full or partial, depending on the timing of the request. If enrollment is not confirmed due to lack of spots, payment failure, unmet requirements, or absence of contract signing, no charges will be incurred.

 

9. Protection of Personal Data
The personal data provided in the enrollment request form will be used exclusively for administrative purposes and to assess the request. The organization is committed to protecting the privacy and security of this data, in accordance with applicable data protection laws.

 

10. Changes to the Terms and Conditions
The organization reserves the right to modify these Terms and Conditions at any time. Any changes will be published on the website or request platform and will take effect immediately after being posted. The Student or Guardian should periodically review these Terms and Conditions to stay informed of any changes.
